This service allows Malaysian citizens to replace a MyKad that has been lost. The Jabatan Pendaftaran Negara (JPN) is the authority responsible for handling all MyKad replacement applications.
Who Is Eligible
Malaysian citizens whose MyKad has been lost are eligible to apply for a replacement.
Required Documents
Police Report: A police report is mandatory for all lost MyKad applications. For the first loss due to a crime (e.g., theft, robbery, snatch theft) or for the second loss and subsequent losses due to negligence.
Copy of MyKad or KPPK 09 Form if available.
Supporting Documents for Address: May be required for verification purposes or if there are changes to your address (e.g., utility bills, driving license).
Step-by-Step Procedure
1. Obtain a Police Report: Lodge a police report detailing the circumstances of the MyKad loss. Ensure the report specifies whether the loss was due to a crime or negligence, as this impacts the fee structure.
2. Determine Application Method:
- Online Application: Visit gantianmykad.jpn.gov.my. This option is available if there are no changes to your photo or personal particulars.
- In-Person Application: Visit any JPN counter nationwide if there are changes to your photo or particulars.
3. Submit Application: Fill in the application form online or present all required documents at the JPN counter.
4. Make Payment: Pay the prescribed replacement fee according to the number of previous losses. Online payments via FPX.
5. Collect MyKad: The new MyKad must be collected in person at a JPN counter. Bring your payment receipt for verification.
Cost & Processing Time
First Loss: RM110 (RM10 application fee + RM100 processing fee)
Second Loss: RM310 (RM10 + RM300)
Third Loss and Subsequent: RM1,010 (RM10 + RM1,000)
Processing Time: Typically 0-7 working days.

Important Notes
