Registering a death is a legal requirement in Malaysia. The process must be completed within 7 days of the demise to obtain the official Death Certificate (Sijil Kematian) issued by the National Registration Department (JPN) and the burial permit required by cemeteries.

Who Is Eligible

  • Relatives, next of kin, or representatives of the deceased individual who passed away inside Malaysia.

  • Deceased's original MyKad or Passport.

    1. Obtain Death Slip: - Hospital Death: The medical officer certifies the cause of death and issues Borang 14. - Home Death: Lodge a report at the nearest police station. The police medical officer will issue the death slip. 2. Burial Permit: Obtain the Burial/Cremation Permit (Permit Menguburkan) from the police station or hospital. 3. Visit JPN Counter: Bring all documents to the JPN office within 7 days. 4. Submit JPN.LM02: Complete the JPN.LM02 registration form. 5.
