Late Registration of Birth is the process of registering a birth in Malaysia after the mandatory 60-day period from the date of the child's birth (previously 14 or 42 days, now standard 60 days). It requires a verification interview and panel approval to prevent identity fraud.

Who Is Eligible

  • Parents of a child born in Malaysia whose birth was not registered within 60 days of delivery.
  • The application can be made by the parents, legal guardian, or the individual themselves (if they have reached adulthood).
  • Required Documents

  • Form JPN.LM01: Completed birth registration form.
  • Late Registration Form: JPN.LM12 completed form.
  • Confirmation of Birth: Hospital birth card or notification of birth. For home births, a confirmation letter from the attending midwife or village head (Penghulu).
  • Parents' Marriage Certificate: Original and copy.
  • Parents' Identity Cards: MyKad or passports.
  • Witness Documents: Testimonials from 2 close relatives who can verify the birth.
  • Step-by-Step Procedure

    1. Visit JPN: Head to the JPN District Head Office (not all small branches process late births). 2. Submit Documents: Submit forms, hospital records, parents' MyKads, and witness statements. 3. Interview: Parents and witnesses must undergo a physical interview with the JPN Registrar to verify the date, time, and location of the birth. 4. DNA Testing (Optional): JPN may request a DNA test (paid by parents) if there is insufficient documentary proof of parentage. 5. Panel Review: The application is forwarded to the JPN Late Birth Committee for approval. Once approved, the birth certificate is printed.
