What Is It

A Certified Extract (Cabutan Sijil) is an official copy or duplicate of birth, death, or marriage registry certificates issued by the National Registration Department (JPN). It is legally equivalent to the original document and is used to replace lost, damaged, or faded certificates.

Who Is Eligible

  • Owner of the Document: The person named in the certificate.
  • Family Members: Parents, spouse, children, or siblings (with proof of relationship).
  • Legal Representative: A lawyer representing the owner (requires a formal letter of authorization).
  • Required Documents

  • Application Form: JPN.LM12 (for birth/death extract) or JPN.KC09 (for marriage/divorce extract).
  • MyKad/Passport: Original and copy of the applicant's identity card.
  • Letter of Authorization: (If applying on behalf of someone else) Authorisation letter and copy of the owner's ID.
  • Copy of Original Certificate: (Optional but helpful) A photocopy of the lost certificate or document number.
  • Step-by-Step Procedure

    1. Visit JPN Counter: Head to any JPN branch office nationwide (counters at UTC are open on weekends). 2. Fill Form: Complete the JPN.LM12 or JPN.KC09 application form. 3. Submit & Pay: Present your MyKad, form, and supporting documents to the officer. Pay the search and extraction fee. 4. Collection: Collect the new certified extract printed on official JPN secure paper. In most cases, it is printed immediately.
